Insurance forms are due by February 1, 2012

Each participating church sending youth and adults to Flower City Work Camp is required to submit a Certificate of Insurance showing a required minimum policy limit of $1,000,000 coverage against any injuries.  The dates of the activity should be listed (Flower City Work Camp 2012 runs from April 9-13, 2012)  and the certificate should specify that it is for coverage of youth and adults participating in the Flower City Work Camp. All Insurance forms should be sent to Claudia Zuch, 41 Coburg Street, Rochester, NY 14612.

Details for East Camp participants:

For churches participating in the East Camp, Browncroft Community Church must be named as the additional insured.  The address for the church is 2530 Browncroft Boulevard, Rochester, NY 14625. 

Details for West Camp participants:

Churches participating in West Camp need to provide two Certificates of Insurance because of the showering arrangement with the Greece Central School District.  One Certificate should name Hope Lutheran Church, 1301 Vintage Lane, Rochester, NY 14626 as the additional insured. 

The second Certificate of Insurance should name the Greece Central School District, Facility Operations and Planning, P.O. Box 300, North Greece, NY 14515-0300 as the additional insured.
